General Information

What are your office hours?
Our office is open Monday–Friday, 8:00 AM–5:00 PM. We are closed on most major holidays. Labs are available select Saturdays, but appointments are always required for labs.

How can I schedule an appointment?
You can call us at (504) 613-0315 or schedule online through our website by logging in as a Member.

Do I need to complete forms before my appointment?
Yes. When you enroll or schedule an appointment, you’ll receive required forms by email with instructions for completion and return.

Can I eat or drink before my appointment?
For annual fasting labs, please avoid food for 10 hours before your visit. Water is encouraged — aim for at least half your body weight in ounces (for example, 150 lbs = 75 oz of water daily, especially in the 24 hours before labs). Continue prescribed medications unless they require food; in that case, wait until after your blood draw.

Can I exercise before a lab appointment?
If you’re having metabolic function testing (recommended every 6–12 months), avoid vigorous exercise for 24–48 hours before your labs.

What should I bring to my appointment?
Please bring a photo ID and a list of your current medications, including supplements.

Can I get labs without a provider visit?
Members can schedule lab appointments through their membership portal.

For non-members, labs cannot be scheduled without booking and paying for an in-person visit prior to booking a lab appointment.

What services do you offer?
We provide direct access to your provider plus a wide range of services, including:

  • Preventive and primary care

  • Chronic disease management

Do you offer telehealth services?
Yes, most services are available via telehealth. Please call our office to confirm if your condition is eligible.

I’m a member; how do I get labs?
Schedule a lab appointment through your member portal and be sure to specify the reason why you’re seeking labs (i.e. STI screen, routine PrEP labs, etc.) when prompted. If not using insurance, payment will be required at time of visit.

Insurance & Billing

What insurance plans do you accept?
Currently, we accept Medicare and commercial plans (Aetna, BCBS, Cigna, Humana, and UHC). Medicaid is accepted only for patients on PrEP or HIV treatment. We are switching to Membership and cash-pay.

Important: As of January 1, 2026, we will no longer accept insurance and will fully transition to a Direct Primary Care membership model.

How can I pay my fees?
Membership dues, medication/supplement costs, and balances can be paid online through the patient portal, by mail, or in person.

Can I use insurance for labs?
Members must indicate at time of booking labs whether or not they’d like to do cash-pay or utilize insurance.

Patient Care

What if I need medical advice after hours?
Send a message through the patient portal or text/call our main office number. The provider will respond by the end of the next business day. For urgent needs, please go to the Emergency Room. Please do not call or text the provider’s personal cell phone unless you’ve been given permission.

Members will have access to their provider’s direct cell number and all messages sent to that number will be stored in the member’s HIPAA-compliant medical chart.

How do I request a prescription refill?
Members may request refills by texting or calling the provider.
